Patient Information
We need some information to grant you a MyChart account. After you submit this request, it may take up to a week for processing and verification. When you're approved, you will receive an email or a letter with your activation code and instructions on how to activate your MyChart account. You must be 18 years old to request a MyChart account through the self-signup workflow.
If you are between 12-17 years old, Click here for the MyChart Access Application. Access may also be requested verbally over the phone by calling 319-356-2555, Monday-Friday 8:00 am-5:00 pm; or 800-777-8442, evenings and weekends. Patients with a mobile phone (that can receive text messages) will automatically receive a text with a MyChart activation link upon scheduling or check in. Patients without a mobile phone will automatically receive an email (if available) with a MyChart activation link upon scheduling or check in.
If you are trying to gain access to a minor child’s record, Click here for the MyChart Parental/Guardian Access Application. Access may also be requested through the parent/guardian’s personal MyChart account. Once logged into the MyChart account, from the Menu select Request Family Access.
If you are trying to gain access to another adult’s record, click here for the MyChart Adult/Adult Access Application. Access may also be given by the patient through the patient’s MyChart account. Once logged into the MyChart account, from the Menu select Sharing Hub, Give friends and family access, then Invite Someone To View Your Record.
The specific information needed to apply for a MyChart account is listed below. In the next step we will verify your identity using questions from a third-party verification system. If we are able to verify your identity, you will be prompted to create a MyChart account. MyChart accounts are available for both current patients and non-patients of University of Iowa Health Care. If we are unable to verify your identity, additional instructions will be provided.
